Mobile App - Forms does not have access to my Photos

Problem:

When I try to attach a photo from my iPad’s Photo Library, I get the error message:

This app does not have access to your photos or videos.

Cause:

That is due to a setting in iOS.

Resolution:

Check the following settings on your iPad:

• Settings > Privacy > Photos > Forms

and ensure that photo library access is turned ON.
